I, the participant and parent/guardian of the participant, hereby acknowledge that I am voluntarily participating in the Mama & Mini sessions organized by Collab London. These sessions involve various physical activities and advice from local professionals conducted at multiple locations. I understand and accept the nature of these activities and the inherent risks involved.

I acknowledge that participation in Mama & Mini involves physical activity which may include movement, exercise, and other activities that could result in injury.

I acknowledge that participation in Mama & Mini involves presentations from trusted local professionals which may include health and wellness advice.

I voluntarily assume all risks associated with participation, including but not limited to falls, physical injury, or other health-related issues that may arise during or after the sessions.

I hereby release, waive, discharge, and covenant not to sue Collab London, its instructors, employees, volunteers, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my child while participating in the Mama and Mini sessions whether caused by negligence or otherwise.

I certify that I am 18 years or older.

I certify that I and/or my child are in good health and have no medical conditions that would prevent safe participation in the Mama and Mini sessions.

I agree to inform the instructors of any relevant medical conditions or allergies prior to participation and seek medical treatment from a licensed medical personnel if needed.
